Before You Begin
Use this form to request information about whether MERP has a claim related to a deceased property owner. A completed and signed authorization is required to submit your request.

What you will need
- Your contact information and company or firm name, if applicable
- The deceased owner's full name and date of death
- The deceased owner's Medicaid ID or Social Security number
- The complete address of the property
- Information about the person authorizing MERP to release claim information
- A completed and signed MERP Authorization and Certification form to upload with your request
Important
You do not need an existing MERP case number or access code to submit this request. Before submitting, you must upload a completed and signed MERP Authorization and Certification form. MERP staff will review the request and authorization before completing the certification.
Your information is secure. Your session expires after a period of inactivity. Your request is not submitted to MERP until you complete the final Review and Submit step.
